Nutrition services may be covered in full or in part by your health insurance or employee benefit plan. It is your responsibility to check with your insurance plan to verify your coverage. Participation as an in-network provider with your insurance plan does not guarantee your visit(s) will be covered. All services not covered by insurance are your financial responsibility.

- Does my plan include any nutrition counseling benefits covered under the preventative care portion of my plan?

- Do I have any out of network coverage for nutrition counseling?
- Are there any limitations on which conditions are covered? (Ask if your specific medical condition or reason for seeing a nutritionist is covered.)
- What is my deductible and has it been met?
- How many visits per year does my health insurance cover?
- What is the coverage amount per nutrition counseling session?
- Is prior approval/referral required from my primary care physician?
