To schedule an appointment, we require a valid credit card be stored on file, to secure.

At Erin Burch Nutrition PLLC, our Registered Dietitians are by appointment only. A last minute cancellation or no show, for any reason, is a time another patient could have used. If you must cancel or reschedule, please do so prior to 48 hours of your appointment time to avoid a missed appointment charge.

Missed Appointment Charge: Your card on file will be charged $50.00 for a no-show or last minute cancellations.  As we understand things can come up last minute, we allow for one last minute reschedule with no charge, if appointment is rescheduled within 24 hours.

Insurance Disclosure Policy
Please note: As per your insurance plan: the quoting of benefits is not a guarantee of payment and the client is responsible if the claim is denied.
